There are three payment levels for Add-A-Form: Basic, Professional 1, and Professional 2. The Add-A-Form Basic is FREE!!! With Basic you get all the features to create your forms and summarized results from all your respondents. With Add-A-Form Professional 1 and Professional 2 you can have access to the raw data collected by your forms and a greater number of stored forms at any time. You also have a technical support line to answer your questions.
