Welcome to the Add-A-Form Wizard, the Wizard will take you step-by-step
through the process of making an on-line form through Add-A-Form.
Each of the steps you will be led through will contain detailed
instructions and a description of the information you are being asked
to enter. There will be examples, and if you do not understand any
terms simply click the glossary to see definitions.
The Wizard consists of two distinct phases. The first phase is entering
in general information that will be applied to your new form, and
the second phase will direct you in adding individual questions to the form.
At the end of the wizard you will have a fully functional form that
is already prepared to take input from users!
