Add-A-Form.com reserves the right to delete forms at our discretion. All "adult", x-rated and profane forms will be deleted immediately. Generally speaking, forms that have been inactive (zero responses) for more than 2 months will be deleted. Generally speaking, Add-A-Form will attempt to contact you before deleting any form that seems to be in use (active in the last year). Add-A-Form users may contact Add-A-Form with specific requests to maintain any specific form(s). Add-A-Form will make every possible attempt to maintain any form(s) so requested in advance by its users.
